So.... I am extremely terrified of jumping?

Back in 2008, (I was 13 years old) I was riding my lease pony at a pony club rally day. She was a 4 year old mare, green broken, and I wasn't very experienced with green horses. It was my turn to do the C grade show jumping round (I was confident) and on the 4th jump, my pony decided the jumps were too scary. So just as she was meant to jump, she bucked twice, (i managed to hold on) jumped the jump SIDEWAYS! Reared up a little, bucked a few more times and then I finally flung off. I broke my hip and was on crutches for 6 months. I still ride of course, but I am too scared to even THINK about going over jumps that are like... 20cm high. I am petrified. I was just wondering if there is any way I can get my confidence back, and maybe any suggestions on what I could have done wrong in the accident, Thanks!
